Short questionnaires, each one a standard instrument used in clinical practice and reproduced here word for word. Most take a couple of minutes. Nothing you enter is stored, sent anywhere, or seen by anyone at the practice.

None of it is written by the practice, adapted or shortened, and none of it is a diagnosis or a clinical assessment. It is a starting point for a conversation. What a result does and does not mean

A number is a prompt, not a verdict

These questionnaires were built to help clinicians and their clients notice change over time. A score does not decide what is wrong and two people with the same total can be having completely different experiences.

What a result is good for is starting a conversation. It can put a word to something that has been hard to name, or show you that what you have been carrying is heavier than you had let yourself think. If the number surprises you, that is worth talking about with someone.

If anything here worries you, that is reason enough to get in touch. This is an appointment-only practice and not a crisis service, so if you need urgent help use one of the routes below.
